As an
advanced practice nurse, your scope of practice widens beyond the individual patient to include whole populations. You will be expected to assess the population wholistically, understanding that factors such as the
social determinants of health (SDOH) do impact the health outcomes of an entire population. Knowing how to identify existing inequities and determining their root causes is necessary to addressing population health.
Navigate to the
National Healthcare Quality and Disparities Reports (NHQDR)
. You will use this site to identify a disparity within your state that is afflicting a specific racial or ethnic group.
Instructions for Using the NHQDR Site
1. On the
NHQDR site
, under Explore the NHQDR Data Tools, select the
State section, then immediately below that select the
Race & Ethnicity tab. The State Benchmark Dashboard: Race & Ethnicity will appear below.
2. On the left side of the dashboard are the filters, select your home state from the dropdown menu.
3. The dashboard will populate with achievement results based on race and ethnicity. The results are broken down into 3 categories of achievement. The first on the far right are the number of indicators that have achieved the benchmark. In the middle are the number of indicators that are close to achieving the benchmark. And the section on the left shows the number of indicators that are far from the benchmark. Select a race or ethnicity that has many indicators far from achieving benchmark. In the filters on the left, select that race or ethnicity on the dropdown menu titled
Population Group.
4. For the Benchmark filter, select
Far from the dropdown menu. This will update the View Underlying Data section for the selected race or ethnicity on the page.
5. The measures on the View Underlying Data section represent disparities—the areas in which the race or ethnic group in your state performs below the benchmark.
Select a measure to use for this assessment.
Analyze all factors that contribute to its existence of the disparity, which will mean looking at all 5 SDOH domains:
. Economic stability
. Education
. Health and health care
. Neighborhood and built environment.
. Social and community context
