Background and Organization Gap Analysis

The Background section includes the detailed evidence of the problem. Should come from project charter. Discusses the organizations practice for the practice problem before implementation of the project.

Up to one page, excluding tables and figures, which will be placed in the appendix.

Significance of the Practice Problem

Includes the PICO question (For the ancillary support primary care clinic staff (P), how does the implementation of a fall risk management protocol (I) compared to current practice (C) affect the fall rates (O) over 12 weeks (T)?) and a narrative to support. Also comes from charter. Provides information on the significance of the problem, such as the following: incidence and/or prevalence, national data and quality/regulatory expectations, impact on patient, family, and health care system, financial implications, cultural, health care policy, quality, safety, legal, ethical implications if pertinent.

One-half to one page.

Summary of the Evidence

This should be an abbreviated, well-written summary of the evidence found to support the project in the previous assessment. This is a brief report of the synthesis of evidence.

One-half to one page long. Must be directly related to the problem and the intervention.

Quality Improvement Framework

Discussion of the framework chosen (The Plan-Do-Study-Act (PDSA)), why, background of the framework in relationship to the problem. One-half to one page long.

Project Description

Discuss the project setting. Discuss the inclusion/exclusion criteria or the population/sample. Describe the intervention such that others can reproduce your project in another geographic location or for another population. No more than one page
